We keep these forms on file for 6 months and will notify you if any animals meeting your criteria become available. This form can be emailed to adoption@earps.org or mailed to the address below. The volunteer application is the form to fill out if you are interested in becoming a volunteer for EARPS. Simply fill it out and email it to sarah@earps.org or mail it to the address below. We have several volunteer opportunities available. |

The critter placement program form is the form to fill out if you wish to have help with finding a new home for your pets yourself without surrendering them. There is a small donation of $5.00 to utilize this service. If you wish to participate in the Critter Placement Program, please fill out this form and email it to surrender@earps.org or mail it to the address below. Please note that we will not process your information until your donation has been received. Filling out this form does NOT add you to our surrender waiting list. You do not need to fill out this form if you wish to be added to our surrender waiting list and participate in the CPP at the same time...please see the surrender waiting list form below. |
